Collagen Supplements: What the Evidence Actually Supports
Walk into almost any pharmacy, co-op, or grocery store from Portland to Portsmouth and you’ll find a tub of collagen powder parked right next to the whey protein. It’s pitched to the people who hike the White Mountains, paddle Casco Bay, and would very much like their knees to keep cooperating into their seventies. The promise is appealing and simple: scoop this in your coffee, rebuild the stuff your body is losing. So the fair question for anyone reaching for it is whether the scoop earns its spot.
Why your body makes less collagen as you age
Collagen is the most abundant protein in the body. It’s the scaffolding in skin, tendons, cartilage, and bone. Production slows with age, which is part of why skin thins and joints stiffen over the decades. That much is real biology, and it’s the kernel of truth the marketing is built around.
The catch shows up the moment you swallow it. Your digestive system treats collagen like any other protein. It gets broken down in the small intestine into amino acids and short peptides, and those building blocks go wherever the body needs protein at that moment, not on a delivery run to your face or your knees. Harvard’s Nutrition Source puts it plainly: there’s a lack of research showing that eating collagen directly benefits skin or joint health. A 2023 review in Heliyon describes the same digestion, with one wrinkle worth noting. A few small peptides, such as one called Pro-Hyp, do appear to survive intact and turn up in the bloodstream. Whether that trickle is enough to matter clinically is exactly what the trials are trying to answer.
What the skin research shows
This is collagen’s strongest column, and it’s a real one. A 2023 systematic review and meta-analysis in Nutrients pooled 26 randomized controlled trials covering 1,721 people and found that hydrolyzed collagen improved skin hydration and elasticity compared with placebo. The effects were statistically significant, and longer use (past eight weeks) worked better for hydration than shorter stints.
Before you clear a shelf, read the fine print the authors themselves flagged. They rated several of the included studies as carrying risk of bias, pointed to missing outcome data in roughly half of them, and called for large-scale trials to firm up the picture. Most of the products tested were proprietary commercial peptides, which matters more than it sounds, and we’ll get to why. What the joint and bone research shows
Here the evidence is busier and a little messier. A 2023 meta-analysis in the Journal of Orthopaedic Surgery and Research looked at four trials and 507 people with knee osteoarthritis and found collagen peptides beat placebo for pain relief. Encouraging, except the same authors graded all four underlying trials as high risk of bias. Good signal, shaky foundation.
The newer work is larger. A 2024 review in the British Journal of Sports Medicine pooled 870 participants across 11 trials and reported overall improvement in both pain and function. A separate 2024 analysis in Osteoarthritis and Cartilage went bigger still, pooling 35 trials and 3,165 patients, and landed on small-to-moderate effects for pain and function. Small-to-moderate isn’t nothing, especially if you’re nursing a cranky knee through ski season. It’s also not the dramatic rebuild the ads imply.
Bone is where one study gets quoted constantly. A 12-month trial in postmenopausal women found that daily collagen peptides nudged bone mineral density up in the spine and femoral neck while the placebo group declined, and a marker of bone formation rose too. A genuinely interesting result. Read the funding line, though: the product and part of the money came from a commercial collagen supplier. That doesn’t make the data fake. It does mean a healthy reader keeps one eyebrow raised.
The funding problem nobody puts on the label
That raised eyebrow deserves its own section, because the pattern repeats. Harvard’s Nutrition Source notes that most, if not all, of the research on collagen supplements is funded or partly funded by industries that stand to benefit from a positive result, which makes it genuinely hard to judge how well these products work. The postmenopausal bone study is one example. The skin trials lean heavily on company-made peptides. None of this is a scandal. It’s just the water collagen research swims in, and it’s the single biggest reason to read the headlines with a grain of salt.
On safety, the picture is reassuring and consistent. Across the knee osteoarthritis meta-analysis and the larger 2024 review, adverse events with collagen were no more common than with placebo. So the realistic downside is mostly your wallet, not your health.
The cheaper way to feed your own collagen
Here’s a fact the marketing tends to skip. Your body can’t build collagen at all without vitamin C, which acts as a required cofactor in the process. The NIH lists the recommended intake at modest levels that a normal diet clears without trying, and you’ll find vitamin C in citrus, berries, leafy greens, bell peppers, and tomatoes. Maine winters make fresh local produce scarce for months, but a bag of frozen wild blueberries and a few bell peppers do the same job.
Harvard’s recommendation for supporting your own collagen is refreshingly boring: eat enough protein, plus whole grains, fruits, and vegetables, and get the supporting nutrients like zinc and vitamin C along the way. No scoop required. If you’ve got the discretionary cash, the skin and joint trials suggest a hydrolyzed collagen peptide might add a modest edge, with a clean safety record. Just go in knowing the research is promising, industry-tangled, and short of a slam dunk.
One more reality worth saying out loud. Nobody checked that tub before it hit the shelf. Under the 1994 law that governs supplements, the FDA does not approve products like collagen for safety or effectiveness before they’re sold. Manufacturers are the ones on the hook for safety and honest labeling, and the FDA generally steps in only after something reaches the market. So the burden of being a careful shopper lands squarely on you.
